Documentation
Documentation is important to protect you in case of a question or dispute surrounding various incidents. You will need to include the date, your actions, and the names of those involved. Record facts; omit opinions and feelings. Document incidents in the SIS to keep all data in one place. Examples of events that require documentation include:
- Suspected abuse and/or neglect
- Bullying
- Harassment
- Parent conferences and correspondence (in person, phone calls, email, and/or letters)
- Student accidents
- Medical emergencies
- Disciplinary actions
- Teacher concerns
Some documentation is anecdotal and is used by a teacher to refresh their memory. These are your personal records that should be maintained separately from any other documentation. They often include decisions, conversations, and actions that are potentially controversial.
